Home safety assessment including but not limited to, physical environment, medication management, and ability to provide adequate nourishment and food safe situations. 

Ensure followup medication administration and refills. 

Assist with scheduling in-home providers such as home health aides for personal care, housekeeping, and food/meal prep.

Safe transportation to and from medical appointments and followup communication with healthcare providers to ensure good continuation. 

Assist with initiating existing Long-Term Care insurance

Provide educated recommendations for safety at home and tools to ensure safe living.
